    Who Can Make a Referral?

    Referrals are welcome from anyone in a participant's support network.

    Whether you are coordinating care professionally or reaching out for yourself or a loved one, our team is ready to help you take the next step. We welcome referrals from Support Coordinators, participants, family members, and allied health professionals – and we will guide you through the process from your first enquiry to your first session.

    Referring a participant on your caseload? We provide timely updates, transparent communication, and reliable support workers who align with your participant's goals.

    Participants

    You can self-refer directly to Lanara. If you are self-managed or plan-managed, we will work with you to build a support plan that fits your life and preferences.

    Family Members & Carers

    Looking for support for a loved one? We understand how important it is to find a team you can trust. We are here to answer your questions and guide you through the process.

    Allied Health Professionals

    If you are a psychologist, occupational therapist, or social worker, you can refer your clients to us for dedicated psychosocial or personal care support that complements your clinical work.

    Our Process

    What happens after you submit a referral?

    Our team will review the information provided in your referral form to understand the specific support requirements and ensure we have the capacity to help.

    1. We review your details

    2. We get in touch

    We will contact you within one to two business days to discuss the referral in more detail, answer any questions, and arrange an initial meet-and-greet.

    Based on the participant's personality, interests, and support needs, we will carefully select a support worker from our team to ensure a comfortable and positive connection.

    Everything you need to know making a referral.

    We aim to respond to all referrals within one to two business days. Once we have reviewed your submission, a member of our team will be in touch to discuss the referral in more detail and answer any questions you may have.

    Yes. We welcome referrals from Support Coordinators, family members, carers, and allied health professionals on behalf of a participant. Simply complete the referral form with your own contact details and include any relevant information about the person you are referring.

    You do not need to have everything prepared before reaching out. The referral form asks for basic contact details, the type of support required, and a preferred start date. If you have additional context about the participant's goals or circumstances, you can include that in the Additional Notes field. We will gather more detail during our follow-up conversation.

    We will always let you know as soon as possible if we do not have current capacity. In that case, we will discuss your timeline, add you to our waitlist if appropriate, and keep you updated so you are not left without a clear next step.
